MARKER POLYPS DO NOT CAUSE CANCER

Although serrated polyps unremarkably are associated with colorectal cancer, it turns out that such polyps are themselves non dangerous, according to a Norwegian report released this calendar week in BMJ Gut, a mag on gastroenterology & hepatology.

As a group, individual with serrated polyps accept an increased opportunity of intestinal cancer, on par with persons who accept high opportunity adenomas. Our squad discovered, however, that piddling happens inwards the polyps. This is non where the cancer develops. We believe, instead, that large, serrated polyps may hold upward markers of increased cancer risk," says Dr. Øyvind Holme, main writer of the report as well as PhD researcher at Norwegian infirmary Sørlandet Sykehus.

Objective
Although serrated polyps may hold upward precursors of colorectal cancer (CRC), prospective information on the long-term CRC opportunity inwards individuals with serrated polyps are lacking.

Design
In a population-based randomised trial, 12 955 individuals aged 50-64 years were screened with flexible sigmoidoscopy, patch 78 220 individuals comprised the command arm. We used Cox models to approximate HRs with 95% CIs for CRC alongside individuals with ≥1 large serrated polyp (≥10 mm inwards diameter), compared with individuals with adenomas at screening, as well as to population controls, as well as multivariate logistic regression to assess polyp opportunity factors for CRC.

Results
A full of 103 individuals had large serrated polyps, of which 81 were included inwards the analyses. Non-advanced adenomas were flora inwards 1488 individuals, advanced adenomas inwards 701. Median follow-up was 10.9 years. Compared with the command arm, the HR for CRC was 2.5 (95% CI 0.8 to 7.8) inwards individuals with large serrated polyps, 2.0 (95% CI 1.3 to 2.9) inwards individuals with advanced adenomas as well as 0.6 (95% CI 0.4 to 1.1) inwards individuals with non-advanced adenomas. H5N1 large serrated polyp was an independent opportunity ingredient for CRC, adjusted for histology, size as well as multiplicity of concomitant adenomas (OR 3.3; 95% CI 1.3 to 8.6). Twenty-three large serrated polyps flora at screening were left inwards situ for a median of 11.0 years. None developed into a malignant tumour.

Conclusions
Individuals with large serrated polyps accept an increased opportunity of CRC, comparable with individuals with advanced adenomas. However, this opportunity may non hold upward related to malignant growth of the serrated polyp.
